If you are a new client, please print and complete the forms on this page and bring them to your first therapy session.
On your first session please bring a drivers license or state issued identification card and your insurance cards for the initial appointment.
If you do not fill these out ahead of time please plan to be to your session 30 minutes before the start of your first appointment.

3. Yearly Demographic Updates (only needed for current patients whose paperwork is a year old)
